    • If you are a student member of IPhA, please list and explain any/all personal efforts you have implemented that promote the profession of pharmacy. No more than 500 words.
    • If you have been actively engaged in the ASSET Student Ambassador program, please upload or write in the provided text field a plan for an innovative programming plan that can be implemented in the future. This plan should describe a student service project that promotes student recruitment, retention, and well-being. The plan should be no more than two pages and include proposed objectives, planning timelines, process and budget needs.
    • In no more than 500 words, please explain your interest in and the importance of pursuing a career in community pharmacy.
    • In no more than 500 words, please explain your understanding of a pharmacist's vital role in a Health System setting.
    • In no more than 500 words, please list and explain any/all involvement in Diabetes Advocacy initiatives/educational outreach.
    • In no more than 750 words, explain why you are interested in pursuing a career as an oncology or industry pharmacist. Describe personal and professional development opportunities you completed that influenced your decision to become an oncology or industry pharmacist.
